2010 US Adult Figure Skating Championships
April 13th – 17th, 2010

Bloomington Ice Garden, 3600 West 98th Street Bloomington, Minnesota 55431

Set up to be completed by: Monday, April 12th • Space Size: Approximately 10 ft long by 5 ft wide. A 6 ft table and 2 chairs provided • Cost: $500 for all 5 days of competition Electricity cost: $25 (for all 5 days)

Please return signed registration form along with $100 deposit or full payment and ST-19 form. If partial payment is made with registration form, balance of vendor fee due on March 15th, 2010. An email confirmation will be sent upon receipt of payment. All communications will be via email once registration is accepted. Vendors are also given a quarter-page ad in the program which is included in your vendor fee. IIf you would like to place an ad, please include ad exactly as you would like to see it in the program along with your other documents. We will try to accommodate all requests. Vendors accepted on a first come, first served basis.

Credit card payments are securely processed via Pay Pal. This form has Pay Pal link with specifically for the event-customized amount of dollars per one entry. However, payer can alter the amount by changing the number of instances to which Pay Pal will adjust the payment amount automatically. Pay Pal link will open in separate Internet browser window, which will allow to complete payment and enter Pay Pal reference ((optional) information into form before its submission. You do not need a Pay Pal account in order to pay via Pay Pal.
